The DQ file documents can be created in 2 different ways: Manually Uploaded Documents and Auto-Generated Documents. Each method follows a specific process, offering unique advantages and requirements within the driver application and e-sign process.
Manually Uploaded Documents:
When initiating the new driver application, there's a checkbox in the "consent" section. Selecting the checkbox indicating the addition of a pre-existing driver to your company triggers the requirement for manual document uploads.
In this scenario:
- Every single document within the DQ File page must be manually uploaded.
- The responsibility lies with the company or the individual managing the application process to gather, organize, and upload each requisite document.
Auto-Generated Documents:
On the other hand, if the checkbox indicating the addition of a pre-existing driver is left unmarked, the driver proceeds to the e-sign process.
Upon completion, a series of essential DQ File documents are auto-generated. These include:
- Driver Employment Application
- Safety Performance History Records Request
- Driver Data Sheet
- Pre-Employment Drug and Alcohol Questionnaire
- Waiver of Disclosure
- Driver Certification of Compliance of CDL
- Cell Phone and Text Bans
- Signature of Authenticity
- Consent for Limited Query for Drug and Alcohol Clearinghouse
- Drug & Alcohol Clearinghouse Policy
- Commercial Motor Vehicle Driver Safety Policy
- Receipt of Policy and Consent
